Instruction Librarian (IL) Guide

Use the Library Instruction Request Form to gather information about the session. 

• Fill out the form yourself if possible, using it as a guide to questions to ask and information to gather. Keep in mind that some information may be filled in later by the librarian conducting the session. 
• Mark the session on your shared electronic calendar immediately. Use question marks if the date is tentative or the instructor to be determined.

At the end of a library instruction session, ask students to fill out this brief survey. The survey can be accessed via this link, QR code, or (if necessary) as a paper copy. It can also be embedded in a Course Guide by using the box below.
